How to Qualify
In order to be eligible for this program you must meet the income guidelines listed at the bottom of this page. For an appointment, simply call (385) 468-3718.
- Women ages 18-39 who have never had a Pap test or who have not had a Pap test in the last five years may receive a free Pap test.
- Women ages 40-49 who meet the income guidelines will be eligible to receive a Pap test*, pelvic exam*, clinical breast exam, and a voucher for a free mammogram. A small fee will be accessed based on a sliding fee scale. Please see “Fee for Service” below or call 963-7300 for more information.
- Women ages 50-64 who meet the income guidelines listed below will be eligible to receive for FREE a Pap test*, pelvic exam*, clinical breast exam, and a voucher for a free mammogram. You may also qualify to receive a free health screening that includes: blood pressure, glucose check, cholesterol screening, height and weight measurements, and risk reduction counseling and coaching. This is a new part of our cancer screening program called WISEWOMAN.
- Women ages 65 and older can be seen in our clinics if they have Medicare B or no Medicare. If you are 65 or older and have Medicare, they will pay for one Mammogram each year.
*Women who have had a hysterectomy due to reasons other than cancer may not qualify for a cervical cancer screening.
